Abstract 【Abstract 】 Objective To explore the risk factors for embryonic chromosomal abnormalities in patients with spontaneous abortion. Method A total of 923 patients with spontaneous abortion who visited the Department of Reproductive Medicine, the Second Hospital of Lanzhou University from September 2019 to November 2024 and voluntarily underwent karyotype testing of embryonic tissue chromosomes were retrospectively selected as the research subjects.They were divided into the abnormal group (n=513) and the normal group (n=410) according to the embryonicchromosome test results. The relevant data, embryonic chromosome test results and their distribution characteristics between the two groups were analyzed, and the risk factors for embryonic chromosomal abnormalities in patients with spontaneous abortion were identi?ed.Statistical methods were performed by independent samples t-test, Mann-Whitney U test, χ2 test or Fisher exact probability method, and multivariate Logistic regression analysis. Result Among the 513 patients in the abnormal group, a total of 606 instances of fetal chromosomal abnormalities were detected. The proportions of aneuploidy, structural abnormalities, chimeras, and polyploidy were 59.08% (358/606), 23.43% (142/606), 9.08% (55/606), and 8.42% (51/606), respectively. The average age of women in the abnormal group was higher than that in the normal group [(31.50±4.22) years vs (30.63 ±3.90) years, t=-3.256, P=0.001]. The levels of serum homocysteine (HCY) and 25-hydroxyvitamin D3 [25-(OH)D3] in the abnormal group were lower than those in the normal group [(10.41 ±4.29) μmol/L vs (11.15±6.86) μmol/L, (14.56±8.10) ng/ml vs (15.91 ±8.76) ng/ml, with t-values of 2.003 and 2.426, respectively, all P<0.05]. There were no statistically significant differences in other indicators (all P>0.05). Multivariate Logistic regression analysis showed that advanced maternal age (OR=2.041, 95% CI: 1.469-2.836), decreased HCY level (OR=0.973, 95% CI: 0.947-1.000), and decreased 25-(OH)D3 level (OR=0.978, 95% CI: 0.963- 0.994) were independent risk factors for embryonic chromosomal abnormalities. Conclusion Spontaneous abortion is mainly caused by embryonic chromosomal abnormalities. Advanced maternal age, decreased HCY level, and decreased 25-(OH)D3 level can increase the risk of embryonic chromosomal abnormalities.
