Abstract Objective To investigate the clinical outcome of a “Two in One” urethroplasty which combined with oral mucosa graft and local pedunculated prepuce flap into on urethra. Method We retrospectively analysis 15 patients, who suffered penile hypospadias with severe chordee underwent a “Two in One” urethroplasty, which combined buccal mucosa and local flap for urethral reconstruction from March 2013 to June 2014 in our hospital. Results Of 15 patients, the extension of penis was satisfied and urethral repair was successful. There were respectively 1 patients with postoperative urinary fistula and urethral stricture, and were respectively cured by fistulous neoplasty and urethral dilatation. Follow up 1 year, there are no the penis curved, urethra fistula,urethral stricture and urethral diverticulum. There is a universal urinate for oneself and a good form. Conclusions “Two in One” urethroplasty which combined oral mucosa graft and local flap is very suitable for penile hypospadias with severe chordee, it is less postoperative complications, and experiences based on the “Two in One” urethroplasty worth extending
