妊娠期甲状腺激素不足与妊娠结局的 研究进展

摘要 妊娠期甲状腺疾病是产科常见合并症，目前 已成为围产医学的研究热点之一。妊娠期甲状腺 激素不足包括临床甲状腺功能减退、亚临床甲状 腺功能减退（subclinical hypothyroidism，SCH）和 低甲状腺素血症（hypothyroxinemia，HIA）。妊 娠期临床甲状腺功能减退指促甲状腺激素（thyroid stimulating hormone，TSH）高于妊娠期参考范围 上限，且游离甲状腺素（free thyroxine，FT4）低 妊娠期参考范围下限；SCH是指妊娠妇女血清 TSH水平高于妊娠期特异性参考范围上限，而FT4 水平在妊娠期特异性参考范围内；HIA是指孕妇血 清TSH水平正常，而FT4水平低于妊娠期特异性参 考范围下限，其中甲状腺自身抗体阴性者称为单 纯低甲状腺素血症（ isolated hypothyroxinemia， IH）［1］ 。越来越多的证据表明妊娠期甲状腺激素 不足对母婴健康产生不利影响，本文旨在对妊娠 期甲状腺激素不足与妊娠结局进行综述。
