多囊卵巢综合征肥胖患者与普通肥胖患者营养代谢及人体成分差异比较

摘要 【摘要】目的  探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)肥胖患者与普通肥胖患者在人体成分、营养代谢指标及膳食结构方面的差异。方法 采用回顾性研究方法,选取2019年7月至2025年6月中山大学附属第六医院收治的PCOS肥胖患者(PCOS肥胖组,n=918)和普通肥胖患者(普通肥胖组,n=1 000),使用InBody S10人体成分分析仪测量体成分,收集2组患者的相关血液学指标,并采用半定量食物频率问卷评估食物摄入情况。统计学方法采用独立样本t检验、多元线性回归分析和Pearson相关性分析。 结果  PCOS肥胖组患者的细胞外水分比率显着高于普通肥胖组(0.39±0.02与0.37±0.02,t=15.321,P<0.001);各频段相位角显着低于普通肥胖组(4.81°±0.64°与(30.58±4.83)%,(85.61±25.32)cm2 与(72.41±22.12)cm2,t值分别为9.215、9.871,P值均<0.001]均高于普通肥胖组;骨骼肌质量、基础代谢率[(21.54±3.27)kg与(23.81±3.59)kg,(1 256.00±187.00)kcal 与(1 354.00±203.00)kcal,t值分别为-12.343、-8.761,P值均<0.001)]均低于普通肥胖组。与普通肥胖组相比,PCOS肥胖组患者存在更明显的营养失衡,表现为蛋白质摄入不足[(15.27±3.14)%与(17.62±3.44)%,t=-12.451,P<0.001],脂肪供能比显着偏低[(26.52±5.31)%与(30.01±5.84)%,t= 水化合物供能比偏高[(58.31±6.24)%与(52.41±5.82)%,t=15.322,P<0.001]。结论 与普通肥胖患者相比,PCOS肥胖患者表现出更突出的体成分异常、膳食结构失衡,提示临床需针对性优化相关临床策略。

Abstract: 【Abstract】Objective   To investigate the differences in body composition, nutritional metabolic indicators, and dietary structure between obese patients with polycystic ovary syndrome (PCOS) and obese patients with general obesity. Method The obese patients with PCOS (PCOS obese group, n=918) and obese patients with general obesity (general obese group, n=1 000) who were admitted to the Sixth Afliated Hospital of Sun Yat-sen University between July 2019 and June 2025 by retrospective study. Body composition was assessed using the InBody S10 body composition analyzer, relevant hematological indicators of the two groups were collected, and dietary intake was evaluated usinga Food Frequency Questionnaire. Statistical analyses were performed by independent sample t-test, multiple linear regression analysis and Pearson correlation analysis. Result The extracellular water ratio of patients in the PCOS obese group was significantly higher than that in the general obese group (0.39±0.02 vs 0.37±0.02, t=15.321, P<0.001), while the phase angle of each frequency band was significantly lower than that in the general obese group (4.81°± 0.64°vs 5.35°±0.71°, t=-12.452, P<0.001); the body fat rate and visceral fat area  of patients in the PCOS obese group were higher than those in the general obese group [(33.24±5.12)% vs (30.58±4.83)%, (85.61±25.32) cm2 vs (72.41±22.12) cm2, with t values of 9.215, 9.871, respectively, all P<0.001]; skeletal muscle mass and basal metabolic rate were significantly lower than those in the general obese group [(21.54±3.27) kg vs (23.81±3.59) kg, (1 256.00±187.00) kcal vs (1 354.00±203.00) kcal, with t values of -12.343, -8.761, respectively, all P<0.001]. Compared with the general obese group, patients in the PCOS obse group exhibited a more pronounced nutritional imbalance, characterized by significantly lower protein intake (15.27%±3.14% vs 17.62%±3.44%, t=-12.451, P<0.001), reduced dietary fiber consumption [(12.53±4.31) g/d vs (14.82±5.14) g/d, t=-7.893, P<0.001], a lower proportion of energy derived from fat [(26.52±5.31) % vs (30.01±5.84) %, t=-10.212, P<0.001], and a higher relative energy contribution from carbohydrates [(58.31%±6.24% vs 52.41%±5.82%, t=15.322, P<0.001].  Conclusion Compared with patients with general obese, obese patients with PCOS exhibit more pronounced abnormalities in body composition, unbalanced dietary structure, highlighting the need for optimized and targeted clinical strategies.
