母胎界面蜕膜自然杀伤细胞的功能

摘要 正常妊娠是一个复杂的过程,作为母体的半同 种移植物,胚胎的植入与胎儿的生长发育受到母体 免疫系统的精密调控。随着滋养层细胞的入侵,子 宫内膜在孕激素和雌激素作用下转化为支持胚胎 着床和胎盘生长的子宫内膜间质组织,称之为蜕 膜,进而形成母体和胎儿相互作用的母胎界面[1] 。 在人母胎界面存在多种免疫细胞,占蜕膜细胞的 30% ~ 40%,包括自然杀伤(natural killer,NK)细 胞、巨噬细胞、T 细胞和树突状细胞。人类妊娠早 期,NK 细胞在母胎界面大量增殖,在妊娠第3 个月 达到高峰(占总淋巴细胞的70%),然后在妊娠中期 和晚期数量下降[2-3] 。在小鼠中,子宫NK(uterine NK,uNK)细胞在妊娠第6.5 天出现并增殖,在妊娠 第10.5 ~ 12.5 天达到高峰,随后下降[4] 。现有研 究提示:蜕膜组织居留NK 细胞可能来源于原位干 细胞扩增,而蜕膜中循环的经典NK 细胞可能来源 于外周迁移[5-7] 。蜕膜NK(decidual NK,dNK)细 胞与外周血NK(peripheral blood NK,pbNK)细胞 在表型、功能及调节机制等方面有明显区别。母胎 界面dNK 细胞的异常可能导致不良妊娠结局。
