摘要  正常妊娠是一个复杂的过程,作为母体的半同种移植物,胚胎的植入与胎儿的生长发育受到母体免疫系统的精密调控。随着滋养层细胞的入侵,子宫内膜在孕激素和雌激素作用下转化为支持胚胎着床和胎盘生长的子宫内膜间质组织,称之为蜕膜,进而形成母体和胎儿相互作用的母胎界面[1]在人母胎界面存在多种免疫细胞,占蜕膜细胞的30% ~ 40%,包括自然杀伤(natural killer,NK)细胞、巨噬细胞、T 细胞和树突状细胞。人类妊娠早期,NK 细胞在母胎界面大量增殖,在妊娠第3 个月达到高峰(占总淋巴细胞的70%),然后在妊娠中期和晚期数量下降[2-3]。在小鼠中,子宫NK(uterineNK,uNK)细胞在妊娠第6.5 天出现并增殖,在妊娠第10.5 ~ 12.5 天达到高峰,随后下降[4]。现有研究提示:蜕膜组织居留NK 细胞可能来源于原位干细胞扩增,而蜕膜中循环的经典NK 细胞可能来源于外周迁移[5-7]。蜕膜NK(decidual NK,dNK)细胞与外周血NK(peripheral blood NK,pbNK)细胞在表型、功能及调节机制等方面有明显区别。母胎界面dNK 细胞的异常可能导致不良妊娠结局。
