摘要
患儿男，1 岁，主因“发现左侧腹股沟区可复性包块 1 年”入院。家长述患儿出生后即发现左腹股沟区间断性出现“栗子”样大小包块，哭闹时最明显。查体：双侧腹股沟区不对称，左侧饱满，可触及大小约5 cm×2 cm×1 cm 的包块，实性，质韧，无触痛，透光试验阴性。可手法还纳入腹，压迫同侧内环口不复出现。外环口增大，可容纳成人示指尖通过，双侧睾丸位于阴囊内，大小位置正常。2019 年 9 月入院后腹股沟区超声显示：左侧腹可探及长条形均质低回声包块，哭闹时可延伸至左腹股沟区和阴囊内；包块彩色多普勒血流显像（color doppler flow imaging， CDFI）可见丰富带状血流信号，肿物尾侧可见宽约 3 mm 索带，长约 1.9 cm，索带 CDFI 未见血流信号，见图 1。超声印象：①左侧腹占位性病变（可疑来源于系膜）；②左侧腹股沟斜疝。入院诊断：左腹股沟肿物、左腹股沟斜疝。入院后完善术前检查，拟行腹腔镜探查，根据术中所见再决定手术方式。
