Effect of single-site intracorporeal purse-suturing stitching versus single-port extracorporeal knotting for laparoscopic inguinal inner ring closure in children

Abstract: 【Abstract】 Objective To compare the changes of inflammatory factors in peripheral venous blood between preoperative and postoperative by single-site (trans-umbilical two-port) intracorporeal purse-suturing (IP) and single-port extracorporeal knotting (EK) for treatment of pediatric hydrocele and investigate the difference of the operation stress and recovery of the children with these two methods. Methods From January to June 2019, 52 cases with hydrocele in Dingzhou People's Hospital were randomly divided into IP group (26 cases) and EK group (26 cases). Peripheral venous blood samples were collected before operation and at 3 h and 24 h after operation by enzyme-linked immuno sorbent assay method to detect the level of serum interleukin (IL)-6, IL-10, tumor necrosis factor (TNF)-α. The statistical analysis was performed byindependent sample t-test and ANOVA of repeated measurements. Results There were no significant difference on age, sexuality, side of lesion in two group (P>0.05). The operation time of EK group was shorter than that in IP group. In IP group, serum IL-6, IL-10 and TNF-α levels were compared before operation and postoperative 3 h and 24 h with significant difference (P=0.000); the level of postoperative 3 h was significantly higher than the preoperative level , and the postoperative 24 h was still higher than the preoperative level although there was a decrease. The levels of 3 inflammatory factors were compared with significant difference before operation and postoperative 3 h and 24 h in EK group (P=0.000); the level of postoperative 3 h was higher than the preoperative level, but the postoperative 24 h decreased and close to the preoperative level. There was no significant difference in serum IL-6, IL-10 and TNF-α levels between the IP group and the EK group before surgery (P>0.05). Serum levels of IL-6, IL-10 and TNF-α in IP group were higher than those in EK group at 3 h and 24 h postoperatively (all P<0.01). Conclusions The stress response and surgical injury of EK surgery were less than those of IP surgery to close processus vaginalis tightly in children with hydrocele. The pro-inflammatory and anti-inflammatory protein in EK group decrease faster to normal level than in IP surgery.
