摘要 胎儿期可发生颅脑缺血或出血等脑血管病变。母体、胎儿、胎盘任一方面出现病理状态,都可能导致胎儿颅脑缺血或出血[1]。胎龄或病变持续时间不同,疾病的表现形式也不同 [2]。在胎儿期,单纯的颅脑实质缺血比出血更为常见[3]。但因为毛细血管在缺血病变的再灌注过程中可能会破裂,从而导致出血,故缺血和出血也可能混合出现。胎儿颅脑缺血或出血的大部分病例预后欠佳,在产前诊断中早期诊断非常重要。目前,产前超声是诊断胎儿脑血管病变的一线检查方法。但对于某些超声检查不易明确的情况,可选择胎儿头颅磁共振成像(magnetic resonance imaging,MRI)检查,以提高诊断的敏感性和特异性。现对胎儿头颅MRI在诊断颅脑缺血和出血性病变中的价值进行阐述。
