免疫性疾病患者的妊娠管理及多学科 团队协作现状

摘要 免疫性疾病是累及全身多脏器的慢性进行性疾病，由自身免疫紊乱和失衡造成，包括系统性红斑狼疮（systemic lupus erythematosus，SLE）、干燥综合征、风湿性关节炎等。免疫性疾病临床表现不一，涉及肾脏和全身各个系统，缺乏特异性，往往高发于生育年龄妇女。近年来随着风湿免疫科、产科医疗及监护水平的提升和发展，给该类患者的孕育带来了新的希望，但如果病情控制不理想，仍会影响妊娠结局，危害母儿安全。因此，如何选择合适的妊娠时机、合理进行妊娠期监测，争取更好的妊娠结局，显得至关重要。妊娠后由于母体-胎盘-胎儿体系的建立，同时由于妊娠期禁忌使用免疫抑制剂（如霉酚酸酯、环磷酰胺），增加了疾病诊断及治疗的难度，妊娠期管理往往较非妊娠期更具挑战性。近年来的研究提示，临床上多学科团队协作，可提高疑难疾病的诊治水平和能力。多学科团队协作的诊疗模式在免疫性疾病妊娠后的诊断和治疗中显得尤为重要，加强免疫性疾病患者的妊娠期管理，相关科室密切合作，有助于改善母儿结局和获得良好预后。
