Relationship between semen routine parameters and reproductive hormones and age in men of childbearing period in different regions of China

Abstract: 【Abstract】 Objective To explore the correlation between semen routine parameters and reproductive hormones and age in men of childbearing period in different regions of China. Method The semen routine parameters and reproductive hormone levels of men of childbearing period were analyzed in four regions of China Mainland, including Hangzhou, Lanzhou, Shenyang and Beijing, respectively, with couples in need of childbearing from March 2019 to December 2021 as the study subjects. Semen volume, sperm concentration, total sperm count, progressively motility sperm percentage, normal morphology rate , head deformity rate and sperm survival rate were analyzed. Sperm concentration and motility,calculate total sperm count and the progressive motility sperm percentage were analyzed by Makler method. Evaluation of sperm morphology and viability using the Papanicolaou staining method and eosin-nigrosin staining. Serum reproductive hormones, including estradiol (E2), folliclestimulating hormone (FSH), luteinizing hormone (LH), prolactin (PRL), testosterone (T) were detected by electrochemiluminescence method. Statistical analysis was performed by one-way analysis of variance, t-test and Pearson correlation analysis. Result The total sample size of the included subjects was 1 143 cases, with ages ranging from 20 to 61 years. After grouping of men according to age of childbearing age , the analysis found that: in the 20-≤35 years age group, sperm concentration and total sperm count showed an increasing trend, while in the >35-≤50 age group, they showed a decreasing trend, but the differences were not statistically significant (P values were 0.539, 0.590), and there was no correlation with age (rvalues were 0.221, 0.194). Correlation analysis between age groups and progressive motility showed that the progressive motility sperm percentage in the 35 to 50 age group decreased significantly (P=0.019), and there was a positive correlation with age (r=0.655). There were no significant differences in sperm concentration, total sperm count and progressive motility sperm percentage between the 20-≤35 years and >35-≤50 years in the four regions (P values were 0.142, 0.354, 0.357). Statistical analysis by different region showed that there were no statistically significant difference in semen parameters between the 20-≤35 age group and the >35-≤50 age group in Lanzhou and Beijing (P>0.05). The semen routine parameters in the 35-50 age group in Hangzhou decreased significantly (P<0.05). There were no statistically significant difference in sperm concentration and total sperm count in Shenyang (all P>0.05), but there was a statistically significant difference in the progressive motility sperm percentage (P=0.047). Summarzing the reproductive hormone levels in the four regions showed that E2, FSH, LH, and PRL in the >35-≤50 age group increasad slightly, while T showed no obvious change, but the differences were not statistically significant (all P>0.05).Conclusion Changes in semen routine parameters among men of childbearing period vary across different age groups. In the 20-≤35 years age group, sperm concentration and total sperm count show an upward trend. In some regions, a decline trend in sperm concentration, total sperm count, and progressive motility sperm percentage is observed in the >35-≤50 years age group. Additionally, in four regions, the levels of E2, FSH, LH, and PRL in the >35-≤50 years age group showed a slight increase, while T levels remained unchanged, with no significant differences observed
