Relationship between miR-195, miR-191-5p in peripheral blood of pregnant women and the severity of preeclampsia, and the value of combined prediction of pregnancy outcomes

Abstract: 【Abstract】 Objective To study the relationship between microRNA-195 (miR-195) and microRNA-191-5P (miR-191-5p) in peripheral blood of pregnant women and the severity of preeclampsia, and the predictive value of combined detection of the two indicators for pregnancy outcomes. Method Using a prospective research method, 124 pregnant women with preeclampsia admitted to Chengdu Qingbaijiang District People's Hospital from January 2022 to June 2023 were included in the observation group, and 124 normal pregnant women who underwent prenatal examinations during the same period were selected as the control group. The expression levels of miR-195 and miR-191-5p mRNA in peripheral blood were detected by real-time fluorescence quantitative polymerase chain reaction (RT-qPCR), to analyze the differences in the expressions of miR-195 and miR-191-5p mRNA in peripheral blood of pregnant women with different severity of preeclampsia and pregnancy outcomes. The correlation of miR-195 and miR-191-5p mRNA expressions with severity of preeclampsia and pregnancy outcome, and the efficiency on predicting pregnancy outcome in pregnant women with preeclampsia were analyzed. Statistical analysis was performed by t-test, test, Pearson correlation analysis, receiver operating characteristic (ROC) curve analysis and multivariate Logistic regression analysis. Result The expression level of miR-195 mRNA in the peripheral blood of the observation group was lower than that of the control group (0.49±0.09 vs 1.00±0.12, t=37.860, P<0.05), while the expression level of miR-191-5p mRNA in the peripheral blood was higher than that of the control group (1.65±0.23 vs 1.00±0.10, t=28.860, P<0.05). The expression level of miR-195 mRNA in the peripheral blood of women with severe preeclampsia was lower than that of women with mild preeclampsia (0.35±0.05 vs 0.59±0.07, t=21.430, P<0.05), and the expression level of miR-191-5p mRNA was higher han that of pregnant women with mild preeclampsia (1.99±0.35 vs 1.27±0.28, t=12.730, P<0.05). The area under the curve (AUC) for the combined diagnosis of preeclampsia severity by miR-195 and miR-191-5p was 0.911 (95% CI: 0.803-0.945), with a sensitivity of 89.78% and a specificity of 83.24%, which was significantly better than any single predictor. The expression level of miR-195 mRNA in the peripheral blood of the adverse pregnancy outcome group was lower than that of the favorable pregnancy outcome group (0.28±0.04 vs 0.65±0.08, t=26.060, P<0.05), while the expression level of miR-191-5p mRNA was higher than that of the favorable pregnancy outcome group (2.13±0.38 vs 1.10±0.19, t=20.050, P<0.05). Pearson correlation analysis showed that the expression level of miR-195 mRNA in the peripheral blood of pregnant women with preeclampsia was negatively correlated with pregnancy outcomes (r=-0.689, P<0.05), while the expression level of miR-191-5p mRNA was positively correlated with pregnancy outcomes (r=0.645, P<0.05). The AUC for the combined prediction of preeclampsia pregnancy outcomes by miR-195 and miR-191-5p was 0.954 (95% CI: 0.878-0.987), with a sensitivity of 94.35% and a specificity of 87.67%, which was significantly better than any single predictor. Conclusion The low expression of miR-195 and high expression of miR-191-5p in the peripheral blood of pregnant women with preeclampsia are related to the severity of the preeclampsia and pregnancy outcome. The combination of the two indicators is more effective in predicting pregnancy outcome.
