The application of root cause analysis in the management of wearing wrist band in neonates |

Abstract 【Abstract】 Objective To explore the application of root cause analysis(RCA)in the management
of wearing wrist band in neonates. Methods From October 2015 to March 2016, 1 476 cases in the term
neonatal intensive care unit of BaYi Children's Hospital, The Seventh Medical Center, General Hospital of
PLA were included in the control group without using RCA to manage the newborn wrist band. From April
2016 to October 2017, 1 316 cases were admitted to the hospital and were analyzed for RCA as the study
group. The incidence of wristband defects was compared between the two groups .The statistical analysis
was performed by χ2 test. Results Two root causes were confirmed through further exploration of the
proximal causes. One was the lack of a complete wearing wrist band procedure; the other one was that the nursing staff had insufficient understanding and paid insufficient attention to the importance of neonatal wrist band. The incidence of wearing wrist band defects in study group was significantly lower than that in control group [0.61% (8/1 316) vs 2.85% (42/1 476)], and there were statistically significant differences (χ2 =19.135,P<0.05). Conclusion The application of RCA in the management of newborn wrist band can analyze the underlying causes of defects, so as to make creative measures purposefully to reduce the incidence of nursing defects, ensure the safety of neonate and improve the quality of care.
