摘要 【摘要】 目的  研究在卵胞浆内单精子显微注射(intracytoplasmic sperm injection,ICSI)技术周期中
M Ⅱ期卵母细胞形态及女性的临床等因素对胚胎发育的影响。 方法 选取海军军医大学长征医院生
殖医学中心2019 年1 月至5 月ICSI 周期中M Ⅱ期卵母细胞形态描述的资料和女性患者的临床资料
进行研究,共纳入30 例患者的资料(M Ⅱ期卵子285 枚)。探讨不孕类型、不孕年限、体质量指数、卵泡
刺激素、雌二醇、治疗方案、染色体等女性的临床因素,以及透明带、卵周间隙、极体碎裂、极体形态、细胞形态、中央区颗粒、胞质内颗粒、折光体、滑面内质网、胞质内空泡等M Ⅱ卵母细胞形态对胚胎发育的影
响。采用χ2 检验进行组间比较、采用单因素和多因素Logistic 回归分析临床因素、卵母细胞形态与胚
胎质量的关系,使用R 语言构建Nomogram 分析预测模型,并采用ROC 曲线对模型的预测效能进行评
价。 结果  ①共纳入285 枚M Ⅱ期卵子,其中87.7% 的M Ⅱ期卵子形态有异常,以细胞质异常最为
常见;② M Ⅱ期卵子形态正常组和异常组获得优质胚胎概率比较差异有统计学意义(P=0.004);③多因
素Logistic 回归分析发现不孕类型(OR=2.964,95%CI :1.564 ~ 5.615,P=0.001)和卵子形态(OR=2.992,95%CI :1.368 ~ 6.545,P=0.006)两种女性的临床因素对胚胎质量的影响有统计学意义;④单因素和多因素Logistic 回归均分析发现中央区颗粒(OR=0.244,95%CI :0.091 ~ 0.654, P=0.005)和卵周间隙(OR=0.377,95%CI :0.173 ~ 0.660,P=0.001)两种形态异常与胚胎质量有关;⑤将临床因素和卵子形态,再次纳入Logistic 回归,将P<0.2 的因素纳入并构建Nomogram 预测模型。绘制ROC 曲线,评价预测模型效能(曲线下面积为0.787,95%CI :0.730 ~ 0.844)。 结论 M Ⅱ期卵子中心区颗粒及卵周间隙的异常对胚胎质量的影响较大,ICSI 的原发不孕女性获得高质量的胚胎概率偏低。

Abstract: 【Abstract】 Objective To study the effects of the clinical and M Ⅱ phase cell morphology on embryonic
development in intracytoplasmic sperm injection (ICSI) cycles. Methods The clinical data of female patients treated with ICSI from January to May 2019 in Changzheng Hospital affiliated to Naval Military Medical University and the morphological description of oocyte cells in M Ⅱ phase were selected (30 patients, 285 M Ⅱ phase oocytes). The influence of the clinical factors of female spouse including infertility type, infertility life, body mass index (BMI), follicle-stimulating hormone (FSH), estradiol, treatment scheme, transparent space,peripheral egg clearance, polar fragmentation, polar form, cell morphology, central particles, intracgranulesorigami, and other M Ⅱ follicles on embryo development were investigated. Chi-square test was used to examine the difference in frequency in two groups. Univariate and multivariate Logistic regression were used to analyze the relationship between clinical and oocyte morphology and embryo quality, the Nomogramanalysis and predictive model were constructed using the R language, and the prediction efficiency was evaluated using the ROC curve. Results ① A total of 285 M Ⅱ phase oocytes were included, of which 87.7% of the MⅡ stage oocytes were abnormal in morphology, and the most common was the abnormality of ytoplasm. ② There were differences between normal and abnormal groups of phase M Ⅱ egg morphology in excellent embryo acquisition probability, with statistically significant (P=0.004). ③ Multi-factor Logistic regression analysis found infertility types only (OR=2.964, 95%CI : 1.564-5.615, P=0.001) and egg morphology (OR=2.992, 95%CI : 1.368-6.545, P=0.006) had statistical differences in the clinical factors. ④Single and multifactor Logistic regression analysis found central zone particles (OR=0.244, 95%CI : 0.091-0.654,P=0.005) and perivitelline space (OR=0.377, 95%CI : 0.173-0.660, P=0.001) were related to embryo quality.⑤ Clinical factors and morphology were included in Logistics regression again, and P<0.2 factors were included to construct the Nomogram prediction model. ROC curve was drawn and the efficiency of the prediction model was evaluated (the area under the curve was 0.787, 95%CI : 0.730-0.844). Conclusion The abnormality of central granule and perioocyte space of M Ⅱ phase oocyte cells has a great influence on embryo quality, and the probability of obtaining high quality embryo is low in primary infertile female spouse.
