新生儿坏死性小肠结肠炎病情加重的 危险因素及预测指标

Risk factors and predictors for aggravation of neonatal necrotizing enterocolitis

摘要 发生于新生儿时期的坏死性小肠结肠炎（necrotizing enterocolitis，NEC）是一种以腹胀、呕吐、血便为主要表现、肠管发生坏死性改变的炎症性疾病，尤以早产儿多见。由于新生儿重症监护室的不断改善以及肺表面活性物质临床使用增加等，早产儿的存活率不断上升，NEC的发病率也不断增高，Bell分期越严重的患儿，病死率越高。Buch等发现Ⅰ期患儿病死率为20%，Ⅱ期为36%， Ⅲ期为67%。王维琼等也有类似发现。Ⅲ期患儿部分需要手术，但手术后易出现短肠综合征、肠狭窄、远期神经发育障碍等并发症。因此，明确影响NEC病情加重的因素，监测相关指标，并采取积极有效的治疗措施，延缓或控制患儿病情，对改善预后有重要意义。现对导致NEC病情加重的危险因素及相关预测指标进行综述。
