摘要 1 临床病例
患儿男，10岁10个月，因确诊肾病综合征（primary nephrotic syndrome，PNS）40天，失语伴口角流涎3小时入院。患儿入院前40天因全身水肿确诊为NS（单纯型），予泼尼松足量口服治疗，并予利尿治疗1周，复查尿常规示蛋白（+++），停用泼尼松，予甲泼尼龙0.5 g冲击治疗3天，复查尿蛋白未转阴。肾活检示微小病变型，再予甲泼尼龙冲击治疗3天好转，加用他克莫司0.1 mg/（kg·d） 每天1次，联合泼尼松2 mg/（kg·d）口服抑制免疫治疗。期间随访尿常规尿蛋白持续阳性。此次入院3小时前患儿突发失语，四肢抖动，口角歪斜流涎，持续半小时，予安定、甘露醇、速尿治疗后，拟“NS、高血压脑病”收入院。
