摘要 开放性脊柱裂（spina bifida operta，SBA）是一种严重的中枢神经系统先天畸形，由胚胎发育过程中神经管不完全闭合而引起，是最常见的神经管畸形表型之一［1］，世界范围内人群发生率为1‰ ~10‰［2］。SBA 患者易患后脑疝，经常伴有脑积水、下肢运动障碍、尿便失禁等一系列并发症，需要终身康复治疗，严重影响其心理发育和生活质量［1］。基于二次打击学说，人们开展了胎儿外科与胎儿镜技术对SBA 患者进行产前宫内缺损修补治疗，可减少羊水刺激与流体压力引起的二次损伤，使术后继发的脑积水减少50%，降低了继发性后脑疝的风险，且患者的下肢运动功能有所改善［3］。然而，由于其对于先天性脊柱裂的原发脊髓发育不良并无治疗作用，术后仍有30%~80% 的患儿存在不同程度的神经功能障碍或过早死亡［4］。
