Histological study of Paneth cells and mucosal inflammatory response in septum tissue of type Ⅰ congenital jejunal atresia

Abstract: 【Abstract】 Objective To observe the distribution of Paneth cells and inflammatory response in mucosa tissue of septum from type Ⅰ congenital jejunal atresia. Method Totally 5 septa of type Ⅰ congenital jejunal atresia located in intestinal wall with distance less than 15 cm from Treitz ligament, and adjacent normal intestinal wall as normal control were collected during intestinal resection and anastomosis. Samples in both groups were targeted to engage immunohistochemistry staining and following semiquantitative test to compare the expression of lysozyme, interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α). Image pro plus 6.0 was used to analyze the average optical density. Statistical analysis was performed by independent sample t-test. Result The mucous of septum had relatively less villi malalignment and no classic epithelial crypt was found, no well-developed Panella cells were found at the base of the intestinal glands in the septum tissue. The expression of lysozyme protein in Panella cells of septum tissue (0.110±0.015) was significantly lower than that in normal jejunal specimens (0.350±0.030), and the difference was statistically significant (t=52.760, P=0.001). The expressions of IL-6 and TNF-α in the mucosal layer of the septum tissue (0.058±0.010 and 0.076±0.009, respectively) were significantly increased compared with those in normal jejunal specimens (0.036±0.007 and 0.022±0.004, respectively), and the differences were statistically significant (t=52.760, P=0.012; t=86.520, P=0.001). Conclusion Compared with normal intestinal wall, the septum presents undeveloped characteristic, weaker defensive function and severe inflammatory responses induced by Paneth cells of type I congenital intestinal atresia. The abnormal differentiation of Paneth cells from intestinal stem cell in fetal period may be involved in the formation process of septal tissue.
