体外激活治疗早发性卵巢功能不全的研究进展

摘要 早发性卵巢功能不全(premature ovarian insufficiency,POI)是指女性在40岁前出现卵巢功能衰退,以月经紊乱(闭经或少经)和卵泡储备减少为主要特征。根据2016 年欧洲人类生殖与胚胎学年会发表的临床指南标准:①少经或闭经4个月以上;②连续2次检查(间隔>4周)促卵泡激素(follicle-stimulating hormone, FSH) >25 U/L,即临床诊断为POI [1]。POI在40岁以下女性中的患病率约1%[2-3],POI患者仅有5%~10%的自然妊娠[4],是导致育龄女性不孕的常见生殖内分泌疾病。研究表明,部分POI患者卵巢内仍含有残余原始卵泡[5-6],这些残余卵泡处于休眠状态,不能被激活并生长发育产生雌孕激素,维持正常的月经周期。由于POI患者残余卵泡数目少且对激素敏感性低1,7]常规的辅助生殖技术很难帮助POI患者激活残余的原始卵泡,获得具有妊娠能力的高质量卵子[8]。因此利用捐赠的卵母细胞进行体外受精及胚胎移植仍是治疗POI不孕症的最终方法。
