喂养不耐受早产儿粪便克雷伯杆菌DNA拷贝数变化分析

摘要 【摘要】 目的  探讨喂养不耐受早产儿粪便克雷伯杆菌DNA 拷贝数变化。 方法 采用前瞻性研究方法,纳入2022 年4 月至2024 年4 月复旦大学附属中山医院青浦分院儿科新生儿室收治的胎龄28~37 周且住院时间≥ 14 d 的早产儿作为研究对象;住院后给予常规治疗,观察其喂养不耐受情况;于3、7、14 日龄分别留取粪便标本,采用探针法荧光定量聚合酶链反应(polymerase chain reaction,PCR)技术检测粪便中克雷伯杆菌DNA 拷贝数。统计学方法采用单因素方差分析、Scheffe 方法、χ2 检验和χ2分割方法。 结果  本研究共纳入早产儿128 例,极早产儿组(n=32)、中期早产儿组(n=58)、晚期早产儿组(n=38)喂养不耐受的发生逐渐减少,差异有统计学意义(P<0.05)。14 日龄喂养不耐受的极早产儿组(n=17)与中期早产儿组(n=9)、晚期早产儿组(n=1)比较,差异有统计学意义(P<0.05)。3 个胎龄组患儿中,7 日龄组粪便克雷伯杆菌拷贝数较多,极早产儿组、中期早产儿组、晚期早产儿组患儿粪便克雷伯杆菌拷贝数逐渐减低[(264.91±77.62) copies/μl、(195.02±96.23) copies/μl、(138.82±61.47) copies/μl,P<0.001]。在极早产儿组中,7、14 日龄患儿粪便克雷伯杆菌拷贝数均显著高于中期早产儿组、晚期早产儿组[7 日龄:(264.91±77.62) copies/μl 与(195.02±96.23) copies/μl、(138.82±61.47) copies/μl,P 值分别
为0.010、P<0.001;14 日龄:(123.69±62.23) copies/μl 与(88.23±50.21) copies/μl、(81.55±47.49) copies/μl,P 值分别为0.040、0.030]。喂养不耐受组(n=79)与喂养耐受组(n=49)患儿粪便克雷伯杆菌拷贝数比较,7、14 日龄喂养不耐受患儿粪便克雷伯杆菌拷贝数均显著增加[7 日龄:(297.44±83.19) copies/μl与(261.29±72.04) copies/μl ;14 日龄:(169.81±62.77) copies/μl 与(133.29±51.33) copies/μl,P 值分别为0.020、0.002]。极早产儿组、中期早产儿组和晚期早产儿组喂养不耐受患儿粪便克雷伯杆菌拷贝数均高于喂养耐受组[(227.67±75.40) copies/μl 与(195.02±50.81) copies/μl,(184.90±43.24) copies/μl与(157.05±32.30) copies/μl,(123.99±29.29) copies/μl 与(102.78±22.32) copies/μl,P 值分别为0.014、<0.001、<0.001]。 结论 早产儿粪便克雷伯杆菌拷贝数与喂养不耐受有关;早产儿粪便克雷伯杆菌拷贝数随喂养不耐受发生减少而降低;喂养不耐受早产儿粪便克雷伯杆菌拷贝数显著增高

Abstract: 【Abstract】 Objective To investigate the copy number variations of Klebsiella in feces of preterm infantswith feeding intolerance. Method From April 2022 to April 2024, premature infants with gestational age of 28 to 37 weeks who were admitted to the Department of Pediatrics, Qingpu Branch of Zhongshan HospitalAffiliated to Fudan University with hospitalization time ≥14 d were included in the study by prospectivestudy method. After hospitalization, the premature infants received conventional treatment and was monitoredfor feeding intolerance. Fecal samples were collected at 3, 7, and 14 days of age, and the copy numberof Klebsiella in the feces was detected using probe-based fluorescent quantitative polymerase chainreaction (PCR) technology. The statistical methods performed by one-way ANOVA, Scheffe method, χ2test and χ2 test segmentation. Result A total of 128 premature infants were included in this study,and the incidence of feeding intolerance gradually decreased in the extremely preterm infants group(n=32), the moderately preterm infants group (n=58) and the late preterm infants group (n=38), and thedifference was statistically significant (P<0.05). The extremely preterm infants group (n=17) with feedingintolerance at 7 and 14 days of age were compared with the moderately preterm infants group (n=9) andthe late preterm infants group (n=1), and the difference was statistically significant (P<0.05). Among thethree gestational age groups, the infants with 7 days of age had higher fecal Klebsiella copy numbers,with a gradual decrease observed in the extremely preterm, moderately preterm and late preterm groups[(264.91±77.62) copies/μl, (195.02±96.23) copies/μl, (138.82±61.47) copies/μl, P<0.001]. In theextremely preterm infant group, the fecal Klebsiella copy numbers in infants at 7 days and 14 days of agewere significantly higher than those in the moderately preterm group and the late preterm infant group [7 daysof age: (264.91±77.62) copies/μl vs (195.02±96.23) copies/μl, (138.82±61.47) copies/μl, P values were0.010 and P<0.001, respectively; 14 days of age: (123.69±62.23) copies/μl vs (88.23±50.21) copies/μl,(81.55±47.49) copies/μl, P values were 0.040 and 0.030, respectively]. A comparison of fecal Klebsiella copynumbers between the feeding intolerance group (n=79) and the feeding in tolerance group (n=49) showedthat the fecal Klebsiella copy numbers in infants with feeding intolerance were significantly higher at both7 days and 14 days of age [7 days of age: (297.44±83.19) copies/μl vs (261.29±72.04) copies/μl; 14 daysof age: (169.81±62.77) copies/μl vs (133.29±51.33) copies/μl, P values were 0.020, 0.002]. The copy numbersof Klebsiella in the feces of infants with feeding intolerance were significantly higher than those with feeding tolerancegroup in the extremely preterm, moderately preterm, and late preterm groups (227.67±75.40) copies/μl vs
(195.02±50.81) copies/μl, (184.90±43.24) copies/μl vs (157.05±32.30) copies/μl, and (123.99±29.29)
copies/μl vs (102.78±22.32) copies/μl, respectively, P values was 0.014, <0.001, <0.001).Conclusion The copynumbers of Klebsiella in the feces of premature infants is related to feeding intolerance. The copy numbers ofKlebsiella in feces of premature infants decreased with the decrease of feeding intolerance. The copy numbersof Klebsiella in feces of premature infants with feeding intolerance increased significantly
