摘要 【摘要】 目的  观察音乐疗法(music therapy,MT)、非营养性吸吮(nonnutritive sucking,NNS)、音乐和非营养性吸吮联合疗法(MT+NNS)缓解新生儿股静脉穿刺疼痛的效果。 方法 选择2016 年12 月至2017 年12 月,战略支援部队特色医学中心儿科新生儿病房进行股静脉穿刺、符合纳入条件的120 例新生儿,并随机等分为4 组,即MT 组、NNS 组、MT+NNS 组和对照组,每组各30 例患儿。对照组按常规进行静脉穿刺;MT 组在穿刺前2 min 开始播放舒缓的音乐直到穿刺后6 min ;NNS 组在穿刺前2 min
开始给予安慰奶嘴直至穿刺后6 min ;MT+NNS 组在穿刺前2 min 播放音乐的同时给予安慰奶嘴直至穿刺后6 min。比较4 组患儿的基本信息、新生儿疼痛评分、患儿啼哭开始时间与持续时间。采用方差分析、χ2 检验进行统计分析。 结果  4 组间基本情况比较差异均无统计学意义(P 值均>0.05)。NNS组穿刺中、穿刺后1 min 疼痛评分显著低于对照组,差异有统计学意义[(5.06±1.20)与(6.30±1.05)分,P=0.001 ;(0.97±1.33)与(2.73±0.86)分,P=0.000]。NNS 组啼哭开始时间、持续时间与对照组比较,差异有统计学意义[(4.2±1.3)与(2.5±1.4)s、(26±4)与(110±15)s,P 值均为0.000]。MT 组和对照组、
MT+NNS 组在所有评价指标比较,差异无统计学意义(P 值均>0.05)。 结论 NNS 可减轻新生儿股静脉穿刺疼痛,MT 不能进一步缓解新生儿股静脉穿刺的疼痛。

Abstract: 【Abstract】 Objective To observe the effect of music therapy (MT), nonnutritive sucking (NNS), and combined music therapy and nonnutritive sucking (MT+NNS) on pain relief in neonates with femoral venous puncture. Methods From December 2016 to December 2017, 120 cases with femoral venous puncture and met the inclusive criteria were selected in neonatal ward department of Pediatrics, Strategic Support Force Medical Center of Chinese PLA, and randomly divided into four groups: MT group, NNS group, MT+NNS group and control group, with each group of 30 newborns. The control group was treated with venous puncture as usual. MT group received soothing music from 2 minutes at pre-puncture to 6 minutes
at post-puncture. NNS group sucked with pacifiers from 2 minutes at pre-puncture to 6 minutes at postpuncture.The neonates in the MT+NNS group received soothing music as well as sucking on pacifiers from 2 minutes at pre-puncture to 6 minutes at post-puncture. The essential information of the infants, neonatal pain scale and the onset and duration of the crying were compared in four groups. The statistical analysis was carried out by variance analysis, χ2 test. Results There was no statistical significance in basic information among the four groups (P>0.05). The NNS group was significantly lower than control group inpain scores of during puncture and 1 minute at post-procedure with statistical significance [(5.06±1.20)vs (6.30±1.05) score,P=0.001; (0.97±1.33) vs (2.73±0.86) score,P=0.000)]. The onset and duration of the crying between NNS group and control group with statistical significance [(4.2±1.3) vs (2.5±1.4) score, (26±4) vs (110±15) score,all P=0.000]. All of evaluating indicators were compared in MT group, control group, and MT+NNS group with no statistical significance (all P>0.05). Conclusion NNScan reduce pain in neonates during intravenous puncture and MT can't further relieve pain of intravenous
puncture.
