早发型丙酸血症超低出生体质量儿1 例分析

Case report of early-onset propionic acidemia in extremely low birth weight infant

摘要 患儿男，第3胎第2产，胎龄29+5周，因其母“妊 娠期高血压、双侧子宫动脉阻力增高、胎儿宫内生长 受限”剖宫产娩出，羊水Ⅰ度，Apgar评分7-9-9分， 出生体质量650 g（低于同胎龄平均出生体质量的第3 个百分数）。父母非近亲结婚，否认家族史。入院 查体：体温36.5 ℃，心率150次/min，呼吸65次/min， 血压40/21 mm Hg（1 mm Hg=0.133 kPa），可见吸 气性三凹征，双肺未闻及干湿啰音，余查体未见异 常。入院后予辅助通气、抗感染、纠正凝血功能等治 疗，病情稳定，生后3 d开始鼻饲母乳1 ml/3 h，开奶 后反复胃内潴留及腹胀，更换深度水解奶粉后未见 好转。生后30 d突然出现四肢频繁抽搐（6~8 min发作 1次，每次持续8~10 s）、进行性呼吸困难、肌张力 低下、低血糖、代谢性酸中毒等。予呼吸机辅助通 气、镇静止惊、纠酸、抗感染等对症支持治疗，病 情未见好转，代谢性酸中毒持续存在，结合患儿病 史考虑遗传代谢病可能性大，及时完善血、尿遗传 代谢筛查及基因检测。生后33 d家属要求签字放弃治 疗，患儿当天宣告临床死亡。入院后实验室检查： 感染指标升高、凝血四项异常、代谢性酸中毒，治 疗后复查均正常；多次血常规示：血小板计数减少 ［（17~77）×109 /L］；生后30 d行实验室检查， 血气分析：代谢性酸中毒、乳酸酸中毒；降钙素原 升高；血清淀粉样蛋白、C反应蛋白正常；血培养阴 性。生后14 d腹部平片：部分肠管较多气体。串联 质谱（tandem mass spectrometry，MS/MS）回报：蛋 氨酸、亮氨酸、脯氨酸增高.
