摘要 肺表面活性物质(pulmonary surfactant,PS)是由多种脂质(90%)和特定蛋白质(10%)组成的膜基系统,磷脂是PS 的主要成分并决定了其功能,特别是磷脂酰胆碱(phosphatidylcholines ,PC)约占PS 总质量的60% ~ 70%,磷脂酰甘油(phosphatidylglycerol ,PG)和磷脂酰肌醇(phosphatidylinositol,PI)占PS 总质量的8% ~ 15%;而中性脂质主要包括胆固醇占PS 总质量的8% ~ 10%。表面活性蛋白(surfactant protein,SP)包括SP-A、SP-B、SP-C 和SP-D,其中SP-B 和SP-C 为小分子疏水性表面活性蛋白,主要参与表面张力的调节,SP-A 和SP-D 为大分子亲水性表面活性蛋白,主要参与肺宿主防御功能。PS 主要功能是最小化肺泡气- 液界面的表面张力,并避免肺泡萎陷,同时具有先天性免疫功能,有助于控制炎症和预防肺部感染。PS 缺乏或功能障碍可引起原发性和急性新生儿呼吸窘迫综合征(neonatal respiratorydistress syndrome,NRDS)以及肺部感染等新生儿呼吸系统疾病,是导致新生儿、尤其是早产儿死亡的主要原因之一。但是,目前上述疾病只能通过临床表现及影像学检查等间接方法诊断,缺乏简单、直接的诊断方法。PS 的成分及功能是诊断NRDS的标准,研究者们开发了不同的检测方法,以期应用于新生儿呼吸系统疾病的快速、可靠诊断。本文对此综述如下。
