新生儿瓜氨酸血症偶合红细胞增多症1例

摘要患儿女，生后58 小时，因精神反应差24 小时入院；系第1 胎第1 产, 胎龄39+3 周，经阴道娩出，羊水Ⅲ°污染，出生体重3000 g ；母乳喂养，于入院前24 小时出现呕吐，呕吐物为奶样及黏液样胃内容物，伴精神反应差、肌张力低下。外院X 线胸片：双肺纹理增多、模糊，右下肺纹理可见小条索状渗出影；血常规：白细胞30.4×109/L，血红蛋白（Hb） 256 g/L, 红细胞压积（HCT）72.4% ；头颅超声：脑结构未见异常，外院诊断为红细胞增多症、新生儿肺炎，给予抗感染、扩容及部分换血等治疗，患儿症状
无明显缓解，复查血常规：白细胞16.4×109/L,Hb 219 g/L, HCT 61.0%，为进一步治疗转入我院。
