摘要 新生儿坏死性小肠结肠炎（necrotizing enterocolitis，NEC）为一种获得性肠道炎症性疾病，主要在早产儿或患特殊疾病（如先天性心脏病、败血症等）的足月新生儿中发生，其特征为肠黏膜甚至肠深层坏死，最常发生在回肠末端，结肠和近端小肠很少受累。NEC的高压因素有早产、低出生体重、缺氧（如患有先天性心脏病）、绒毛膜羊膜炎、围产期缺血缺氧性损伤、输血、喂养不当、感染、肠道异常菌群的定植及过度繁殖等。在活产婴儿中，NEC的发病率为1‰~5‰，病死率为20%~56%。早期诊断、及时进行合理治疗是降低NEC病死率、提高存活率的关键。轻症NEC患儿可采取保守治疗，但严重病例治疗难度大，往往需要手术治疗，病死率高，预后差。严重NEC患儿治疗方法以外科手术为主，选择恰当的手术时机十分重要，既要防止在没有手术指征的情况下盲目手术，又要避免因延误手术时机而产生不良后果。现将近年来关于NEC手术指征的研究进展进行综述。
