早产儿肺损伤的预防及新生儿呼吸支持的研究进展

Prevention of lung injury in premature infants and research progress of neonatal respiratory support

摘要 在过去的40年里，新生儿重症监护病房（neonatal intensive care unit，NICU）技术的进步使出生体重更低、胎龄更小的早产儿得以成活。但伴随而来的是远期呼吸功能障碍发生率的增加，主要以支气管肺发育不良（bronchopulmonary dysplasia，BPD）的形式出现。预防肺损伤的发生对于预防BPD及改进早产儿的长期预后非常重要，如何避免呼吸机相关性的肺损伤，也因此成为近年来早产儿临床和科研关注的重点。本文将对早产儿肺损伤的易感性及病理生理、预防肺损伤的通气策略及新生儿呼吸支持的新进展进行阐述。
