摘要重度支气管肺发育不良（severe bronchopulmonarydysplasia，sBPD）的治疗一直是国内外临床的难题^［1-3］，往往很难治疗甚至是无法治疗。美国对sBPD 患儿的治疗理念不统一，暂无很好的理论支持。在过去7 年里，费城儿童医院新生儿及小婴儿重症慢性肺疾病团队通过治疗400 余例sBPD 患儿，收获了较多临床经验。本文由阐述sBPD 病理生理基础出发，通过讨论早产儿呼吸窘迫综合征（respiratory distress syndrome，RDS）与BPD 的生理和肺力学变化，阐述本团队预防与治疗sBPD 的机械通气理念。
