摘要 患儿男，1个月，福利院收养弃婴，主因“脐部包块、双下肢为四肢（左、右下肢各两条）畸形”于2013年5月15日入我院，体格检查：头面部及腹部发育正常，双上肢发育、活动正常，脐部软组织膨出，大小约5 cm×4 cm×3 cm，质软，表面被膜结痂干燥，被膜下可见膨出肠管，腹壁静脉曲张，四条下肢，双阴茎，双阴囊，双侧肛门（图1），外观正常。肛门：右侧肛门可见自主排便，左侧肛门未见排便，肛查棉签头部进入，拔出可见黄色黏液附着。双侧阴茎发育不良，右侧阴囊可及一睾丸，左侧阴囊空虚，双侧尿道排尿正常，表现为同时排尿，盆腔以下皮肤感觉无明显异常，骶尾部包块大小约3 cm×2 cm×2 cm，质软，周围皮肤发青。下肢：双外侧下肢呈屈髋屈膝外展外旋位，自主活动；双内侧下肢短小呈屈膝踝背伸内旋位，无明显自主活动。骨盆CT重建及腹腔超声检查结果提示患儿腹腔内脏器中，膀胱、骨盆（图2），结肠系统，腹主动脉均为两套。诊断：尾端重复畸形：骨盆、下肢、泌尿生殖系统、肛门、直肠；脐膨出；双侧隐睾；肠旋转不良；回肠重复畸形；美克尔憩室。
