摘要 自1983年国内第1批儿童重症监护室(pediatric intensive care unit, PICU)成立至今,目前PICU的数量、规模、硬件设施及从业人员均发展迅速。随着PICU临床实践基地的发展成熟和具有学科特色的理论和技术体系形成,已清晰地标志着儿童重症医学已经成为独立的临床新学科,且因其具有“高技术依赖性”而明显区别于其他临床学科。PICU的出现是医学科学和技术发展到相当高度并结合的产物,其医疗活动绝大部分都是建立在高技术仪器设备的基础上。体外生命支持系统是儿童重症医学的核心技术,高度依靠高技术仪器设备来实现。该核心技术系统由器官功能监测、维护和替代技术组成(图1)。PICU的核心工作就是利用体外生命支持技术,对出现单个或多个器官功能不全甚至衰竭的重症患儿进行支持,争取原发病恢复、诊断或下一步治疗的时间窗口。体外生命支持技术系统的建设水平是PICU水平的标志。
